If you voluntarily provide medical documents or other sensitive information, you acknowledge that such information may be processed and shared where necessary for the requested service and where an appropriate legal basis exists.
4. Accuracy and Authority
You confirm that the information and documents you provide are accurate to the best of your knowledge and that you are authorised to provide information concerning another person where applicable.
5. Withdrawal
Where processing is based on consent, you may withdraw consent. Withdrawal may affect our ability to continue a service where the information is necessary for the requested coordination.
